Form fields tell you what to type

What the source requires

Every consultation form, booking form and newsletter box needs a programmatic label, not just placeholder text that disappears the moment someone types.

Why this is hard to satisfy

Placeholder-only fields look labelled to a sighted user and are silent to a screen reader. A booking form nobody can complete is a lost patient, which is the version of this a practice acts on.

What we do about it

Labels are generated with the field, so a form cannot ship without them.
