What the source requires
Visual structure must exist in the markup, not only in the styling.
Why this is hard to satisfy
Page builders produce bold styled text where a heading belongs, so a screen reader user cannot skim the page by its structure — the way most of them navigate.
What we do about it
Content is authored as structure and rendered to styling, rather than the reverse.
